Rate
My fees are $150 for 90 min initial consultation and $130 for a follow up one hour session.

Reduced Fee
Based on income, reduced fee services are available.

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan.  I am a provider with Aetna; Anthem Blue Cross/Blue Shield; PHCS; Husky A; CHN PPO; Multiplan, CHAMPVA, Tricare.  However, please check your coverage carefully by asking the following questions of your behavioral health division within your particular insurance:

• Does my particular plan include mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is my copay?
• Is approval required from my primary care physician?

Payment
Cash or check are accepted for payment.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
